SWOC Check-In

I give Sportswomen of Colorado permission to edit, caption, publish, and share the video I am submitting, along with my name, sport, school or team, and honor year. SWOC may use it on its website, in its emails, on its social media accounts, and in its own promotional and fundraising materials. The video is mine and I have the right to send it. SWOC will not sell it or license it to another company for advertising. I can ask SWOC to take it down at any time, and SWOC will remove it from anywhere it controls.

I am the parent or legal guardian of the athlete named on this form, and I give Sportswomen of Colorado permission to edit, caption, publish, and share the video she submitted, along with her first and last name, sport, school or team, and honor year. SWOC may use it on its website, in its emails, on its social media accounts, and in its own promotional and fundraising materials. SWOC will not sell this video or license it to another company for advertising, and will not publish her home address, phone number, or email. I can ask SWOC to take the video down at any time, and SWOC will remove it from anywhere it controls. If there is anything we should leave out, such as a school name or hometown, say so in the box above and we will do it.
